Form W9

Request for Taxpayer Identification Number and Certification.  The W9 Form must be completed by all vendors that provide services for your company and are paid $600 or more during the calendar year.

Form I-9

Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States.

Form W-4

The W-4 is an IRS form your employees complete to let you know how much money to withhold from their paycheck for federal taxes.

Form A-4

Arizona law requires employers to withhold Arizona income tax from employee wages for work done in Arizona.  All Arizona employers are required to keep a completed and signed Form A-4 on file for each employee.
